Christian Education for the Community



To enroll for the new school year, your child will need the following items.
  • A completed application form (download here)
  • A completed medical release form (download here)
  • Tuition Deposit: $100.00/child or $150/family (non-refundable)
  • A completed financial agreement form (page 2 of the application) (download here)
  • Receipt of character reference forms – grades 1-2 (2), grades 3-12 (3) (download here)
  • Additional Permissions form (download here)
  • Authorized Pick-up form (download here)
  • Completion of new student interview (student and parent are to attend)
  • First graders and students entering school for the first time are required to show their birth certificates and a copy must be kept on file in their permanent record.
  • Certificate of Immunization Status (Required by the State of Oregon)
  • Achievement Test scores and State Assessment test scores (when applicable)
